- Any relevant diagnostic test results or x-rays.

- Your insurance card and driver's licenses or other form of picture    identification.  We will make a copy of them for your patient chart.

- Most insurance plans require co-payment at the time of service. We   ask that all co-payments and deductibles be paid at the time of   service.  Our office accepts cash, checks, MasterCard and Visa.   Co-payment is expected at the time of service.

- Bring referral forms if required by your insurance.

- Inform our office and your insurance company of any address or   telephone number changes.

- If you are not insured or have financial hardships, our staff will    be happy to work with you to make payment arrangements.
